Key Responsibilities

• Provide physical therapy services to an adult and geriatric patient population, with a focus on strokes and orthopedics in a home health setting
• Conduct an expected 7 patient visits per day with full-time productivity of approximately 25 units
• Document patient care and visit data using Home Care Home Base (HCHB), with a transition to EPIC anticipated around September 2026
• Track mileage through HCHB (and eventually EPIC); mileage is reimbursed at the IRS standard rate
• Float within scope of practice as needed
• Adhere to holiday work expectations: if one or two major holidays fall within the assignment period, the clinician is expected to work them; if three fall within the assignment period, the clinician is expected to work two

Required Qualifications

• Minimum 1 year of experience; first-time travelers are accepted
• Active Georgia PT License or Compact PT License required (verify member state eligibility prior to submission)
• BLS Certification required
• Full compliance with onboarding requirements must be completed within 48 hours of contract signing; all compliance documents are due the Tuesday prior to start
• All requested time off (max 7 days per contract) must be finalized at time of submission; no RTO or schedule requests will be accepted post-offer or on-site, and no RTO may be requested during orientation week
• Local candidates are welcome and accepted

Preferred Experience / Skills

• Experience with Home Care Home Base (HCHB) is highly preferred; clinicians without HCHB experience will be required to complete an online HCHB course prior to starting
• Prior home health experience, particularly with geriatric, stroke, or orthopedic patient populations
• Familiarity with EPIC EMR is a plus given the upcoming system transition
