Key Responsibilities

• Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete thorough medical record documentation in a clear, concise, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities
• Delegate and direct discipline-specific therapy services for assigned patients while ensuring care is provided under valid physician orders and within insurance approvals
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in good standing in the state of NH
• Ability to complete comprehensive patient evaluations and develop clinically sound treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
• Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students in a clinical environment
• Demonstrated involvement in continuing education and professional development activities
