Key Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
Implement therapeutic interventions to improve patients' functional abilities and independence in daily activities
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care

Required Qualifications

Active Occupational Therapist (OT) license in the state of South Carolina
Graduate of an accredited Occupational Therapy program
Eligibility to work in a skilled nursing facility environment

Preferred Experience / Skills

Prior experience working in a skilled nursing facility (SNF) setting
Familiarity with Medicare documentation and compliance requirements
