Key Responsibilities

• Provide occupational therapy services across a split setting: approximately 50% Home Health and 50% Skilled Nursing Facility, with flexibility based on area need
• Evaluate patients and develop individualized treatment plans to improve functional independence and quality of life
• Document patient progress and maintain accurate records using HCHB (Home Health) and NetHealth (SNF) software platforms
• Meet productivity expectations of 90% in the Home Health setting and 85% in the Skilled Nursing Facility setting
• Collaborate with an on-site therapy supervisor and interdisciplinary care team to ensure coordinated patient care

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Minnesota or eligibility to obtain
• Ability to work across both Home Health and Skilled Nursing Facility settings as needed
• Proficiency or willingness to learn HCHB and NetHealth documentation systems
• Must reside at least 80 miles from the facility to be eligible for standard contract rates

Preferred Experience / Skills

• Prior experience in Home Health or Skilled Nursing Facility settings
• Familiarity with productivity-based therapy models
• Strong adaptability and willingness to flex between care settings based on patient and facility need
