Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals aimed at progressing patients toward discharge within an established length of stay.
• Collaborate with the interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
• Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management.
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
• Adhere to discipline-specific state practice acts and regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements.

Required Qualifications

• Active Physical Therapist license in good standing in the state of Virginia (or eligibility to obtain)
• Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
• Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts

Preferred Experience / Skills

• Experience in an inpatient rehabilitation or acute care hospital setting
• Familiarity with interdisciplinary team collaboration and discharge planning processes
• Experience supervising or mentoring PT students or clinical affiliates
• Active involvement in professional development, continuing education, or professional organizations
