Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs and physician direction.
- Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
- Effectively manage individual, concurrent, and group treatment sessions based on patient care needs, demonstrating flexibility in caseload management.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements.
Required Qualifications
- Active Physical Therapist license in the state of Ohio in good standing.
- Ability to complete comprehensive patient evaluations and develop clinically reasoned treatment plans and goals.
- Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
- Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
- Ability to work a schedule that includes weekday and weekend shifts (2 weekends per month).
Preferred Experience / Skills
- Experience working within an interdisciplinary team environment in an inpatient or rehabilitation setting.
- Familiarity with functional quality indicator assessments and letters of medical necessity documentation.
- Active involvement in professional organizations, continuing education, or community advocacy efforts.
- Experience supervising or mentoring Physical Therapy students.
