Contract (13 Weeks) | Days | Monday – Friday | Onsite

Key Responsibilities

• Provide physical therapy or physical therapy assistant services to residents in a care facility setting
• Deliver skilled therapy interventions in alignment with each patient's individualized plan of care
• Collaborate with the interdisciplinary care team to support patient progress and goal attainment
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
• Present valid state or federal issued photo identification upon entry and wear a photo badge at all times while on site

Required Qualifications

• Active and unrestricted New Mexico PT or PTA license
• Valid state or federal issued photo identification required
• Ability to work Monday through Friday day shifts for the duration of a 13-week contract

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care environment
• Familiarity with Medicare documentation and compliance requirements
