Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established goals
Collaborate with interdisciplinary care team to optimize patient outcomes
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

Active Physical Therapist license required
Skilled Nursing Facility (SNF) experience required
Ability to provide a 14-day notice prior to end of assignment

Preferred Experience / Skills

Experience working in a geriatric or long-term care population
Familiarity with MDS documentation and skilled level of care requirements
