Key Responsibilities

• Evaluate and treat patients using evidence-based occupational therapy interventions
• Develop and implement individualized treatment plans to support patient goals and functional independence
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary care teams to ensure coordinated patient care
• Maintain valid state licensure and adhere to all facility compliance requirements, including wearing a photo ID badge at all times while on site

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Colorado
• Graduate of an accredited Occupational Therapy program
• Valid state or federal issued photo identification required
• Ability to comply with all facility credentialing and registry personnel requirements

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Experience with geriatric patient populations
• Familiarity with electronic medical records (EMR) documentation systems
