Key Responsibilities
- Provide physical therapy evaluations and treatments to patients in a home health setting across an assigned geographic pod
- Actively manage a caseload to meet expected weekly productivity targets of at least 30 points per week
- Proactively backfill cancelled visits by contacting patients to reschedule or advance upcoming appointments
- Complete clinical documentation accurately and in a timely manner using HCHB (Homecare Homebase)
- Coordinate with the home office team to maintain a full and productive schedule throughout the week
Required Qualifications
- Active Physical Therapist (PT) license in the state of California
- Minimum 1–2 years of clinical experience as a Physical Therapist
- Proficiency with or ability to learn HCHB (Homecare Homebase) documentation platform
- Valid driver's license and reliable transportation for home visits
Preferred Experience / Skills
- Prior home health physical therapy experience strongly preferred
- Prior experience with HCHB documentation software
- Strong self-management skills and ability to independently manage a field-based caseload
