Key Responsibilities
- Evaluate and treat patients with speech, language, voice, and swallowing disorders in a skilled nursing facility setting
- Develop and implement individualized treatment plans based on patient assessments and clinical goals
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to support comprehensive patient outcomes
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements
Required Qualifications
- Active Speech Language Pathologist (SLP) license in the state of Colorado
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required per facility standards
- Experience working in a skilled nursing facility or long-term care environment
- Strong clinical documentation and communication skills
Preferred Experience / Skills
- Prior experience with dysphagia evaluation and treatment, including modified barium swallow studies
- Familiarity with MDS/PPS documentation processes in a SNF setting
- Experience with geriatric patient populations
