Key Responsibilities
- Evaluate patients and establish individualized plans of care within the home environment, coordinating with other disciplines and securing orders for additional services as needed
- Provide direct patient care based on established care plans and physician orders, applying occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation
- Complete and master scoring of the CMS-required OASIS instrument, collaborating with the Quality team on necessary adjustments
- Oversee services provided by Occupational Therapy Assistants and perform supervisory visits within required timeframes; assume supervision of CNAs when other disciplines are no longer involved in the care plan
- Participate in multidisciplinary conferences and Quality Assurance Performance Improvement initiatives; communicate appropriately with physicians and care team members
- Maintain accurate point-of-care documentation, time logs, and mileage records in accordance with agency policies
Required Qualifications
- Bachelor of Science degree or higher from an accredited Occupational Therapy program
- Current Occupational Therapist license to practice in the State of Montana
- Valid driver's license and appropriate auto insurance
- Working knowledge of Medicare home health regulations and insurance requirements
Preferred Experience / Skills
- Minimum of two years of prior occupational therapy experience
- Familiarity with OASIS documentation and CMS home health Conditions of Participation
- Experience supervising Occupational Therapy Assistants or CNAs in a home health setting
