Schedule / Job Type

Contract | Travel | Days | 13-Week Assignment | Expected 36 hours/week | Shift: 8:00 AM – 4:00 PM, includes some weekends with comp days

Key Responsibilities

• Provide physical therapy evaluations and treatments to home health patients in Lincoln County
• Develop and implement individualized care plans to support patient rehabilitation and functional goals
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to coordinate patient care and ensure continuity of services
• Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards

Required Qualifications

• Valid Maine Physical Therapy License
• BLS/CPR Certification
• Minimum of 2 years of physical therapy experience

Preferred Experience / Skills

• Prior home health or home care experience strongly preferred
• Familiarity with home health documentation and care coordination workflows
