Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM

Key Responsibilities

Provide physical therapy evaluations and treatments to patients in a home health setting
Manage a patient caseload within a 50-mile radius of the office
Meet a productivity expectation of 30 points per week
Document patient care accurately and timely using HCHB (HomeCare HomeBase) software
Develop and implement individualized treatment plans to support patient goals and functional outcomes

Required Qualifications

Active Physical Therapist (PT) license in the state of Kentucky
Previous home health experience required
Proficiency with HCHB (HomeCare HomeBase) documentation system required
Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

Strong time management and self-directed work skills suited for a field-based role
Experience managing productivity standards in a home health environment
