Contract (13 Weeks) | Full-Time | Days | Monday – Friday, 8:00 AM – 4:30 PM | Onsite
Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting
- Manage a caseload of approximately 28 points per week
- Document patient care and progress using HCHB (Homecare Homebase)
- Complete a minimum of 2 days of in-office orientation during the first two weeks
- Participate in optional weekend on-call rotation, approximately once per month
Required Qualifications
- Active Physical Therapist (PT) license in the state of Connecticut
- Prior home health experience required
- Proficiency with HCHB (Homecare Homebase) documentation system required
- Must reside within 40 miles of the Watertown, CT office to be eligible
Preferred Experience / Skills
- Experience managing productivity-based caseloads in a home health environment
- Familiarity with home health regulatory and compliance standards
