Contract | Full-Time | Days | 5 x 8-hour shifts per week | Rotating weekend and holiday schedule

Key Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily patient visits in a home health setting
• Manage an expected caseload of 6.5 points per day
• Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
• Travel to patient homes within the assigned coverage area to deliver skilled physical therapy services
• Participate in rotating weekend shifts (expected hours 8:00 AM – 5:00 PM) and holiday rotation
• Utilize a company-provided phone and tablet for documentation and communication while in the field

Required Qualifications

• Active Physical Therapist (PT) license in the state of Kentucky
• Current BLS certification required
• Ability to independently manage a home health caseload across an assigned geographic area
• Willingness to participate in rotating weekend and holiday schedules

Preferred Experience / Skills

• Prior experience in a home health setting
• Experience treating patients with CHF, orthopedic, or pulmonary diagnoses
• Candidates with at least some clinical experience preferred; new graduates are not preferred for this role
