Key Responsibilities
- Provide direct physical therapy services to patients in a skilled nursing facility setting
- Evaluate patients and develop individualized treatment plans to address functional deficits
- Implement and progress therapeutic interventions in alignment with patient goals
- Collaborate with interdisciplinary care team to coordinate patient care
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
- Active Physical Therapist license required
- Skilled Nursing Facility (SNF) experience required
- Ability to start as soon as possible
- Must adhere to a 14-day notice policy
Preferred Experience / Skills
- Prior travel therapy experience
- Experience working in a fast-paced, high-census SNF environment
