Contract | 13-Week Travel Assignment | Days, 8:00 AM – 4:30 PM | Weekend Rotation Required

Key Responsibilities

• Perform utilization management and revenue integrity functions within a hospital-based Case Management unit
• Coordinate care and facilitate discharge planning for a patient caseload with an 18:1 ratio
• Collaborate with interdisciplinary teams to ensure appropriate level of care and timely transitions
• Document and manage patient information using the Midas EMR system
• Demonstrate leadership qualities while working within a small, focused case management team

Required Qualifications

• Active Registered Nurse (RN) license – New Hampshire RN license required at time of submission
• Active Compact (multi-state) RN license required at time of submission
• Minimum 2 years of acute hospital RN Case Management experience
• Critical care experience required
• BLS certification required
• Business professional dress code adherence required

Preferred Experience / Skills

• Experience with utilization management and care coordination in an acute hospital environment
• Familiarity with Midas or similar EMR platforms
• Strong leadership qualities and ability to work independently within a small team setting
