Description

This position is responsible for providing individualized, goal-oriented nursing care using the nursing process in accordance with a physician's order. The Home Health RN also serves as a Case Manager when leading cases. With a focus on community well-being, you'll deliver exceptional home health and hospice care, ensuring high-quality services to individuals across central Vermont.

Requirements

EMR System knowledge preferred
General Certifications (BLS/BCLS)
State License Details: Must Be Currently Active
1 year of nursing experience required
