Description
In this mobile position, LPNs will support the Transitions of Care program, focusing on reducing readmission rates. Responsibilities include appointment scheduling, biometric data monitoring, and patient education. LPNs collaborate with an RN Care Manager and a multidisciplinary team to implement care plans. The role requires flexibility, strong communication skills, and the ability to maintain frequent patient contact while managing a caseload of approximately 25-30 patients.
