As a Home Health RN at Central Vermont Home Health & Hospice, you will provide individualized, goal-oriented nursing care in patients' homes, ensuring high-quality services. This position primarily involves case management, where you will utilize your nursing skills to coordinate care and support patients' needs. Your schedule will be Tuesday to Saturday, allowing for a balanced work-life experience.
