Description
As a Hospice RN Case Manager, you will provide skilled nursing and care management to patients in their homes. Your role involves assessing patient needs, coordinating services, and ensuring continuity of care. The ideal candidate will work collaboratively with an interdisciplinary team to enhance patient outcomes, maintain accurate records, and participate in quality improvement activities. This position offers a Monday to Friday schedule with on-call duties as needed.
