Description

CASE MANAGER ** MUST HAVE RECENT ACUTE CARE HOSPITAL CASE MGMT EXP ** !! will float between both Walnut Creek and Concord campuses, they are 8 miles apart !!

Will work EOW or a set rotation... the set rotation is working EVERY Tues-Sat

Shift: 8 hour days (0800-1630) - 40 hours per week

Duration: 13 weeks

REQUIRED : Active CA license Must have 2+ years experience in an acute care hospital

PREFERRED : previous travel and EPIC exp
