Description

Requirements:

Education: BSN required
License: Active RN (Michigan)
Experience:
- Minimum 5 years acute inpatient case management experience
- Experience with acute discharge planning
- BLS (AHA) required
- Familiarity with CarePort system required
- Experience with AllScripts, GEMS/Sunrise preferred
- Must be able to work on the floor and adapt quickly
