Our client is currently seeking a Registered Nurse-- Case Manager

 

Location: Lake County, CA (Field-Based) 

Schedule: Day Shift | 40 Hours/Week 

Travel: Required within coverage area (mileage reimbursed at government rate)

Role Overview

As a Home Health RN Case Manager, you’ll lead the coordination of care for patients in their homes, ensuring timely, appropriate, and high-quality services. You’ll serve as a clinical liaison among patients, families, physicians, and interdisciplinary teams, driving optimal outcomes and continuity of care.

What You’ll Do

• Lead interdisciplinary care planning and ensure alignment with clinical needs and covered services
• Monitor diagnostics, treatments, and procedures for appropriateness and timeliness
• Collaborate with physicians on resource utilization, treatment plans, and discharge strategies
• Coordinate transfers to tertiary centers and ensure continuity of care
• Manage complex cases including frequent ED utilizers, chronic pain, substance abuse, and homelessness
• Collect and report data for regulatory and accreditation compliance
• Perform other duties as assigned

Requirements

• Required:
  • Active RN license in California
  • Basic Life Support (BLS, HS-BLS, or RQI BLS) certification
  • Home Health experience
  • Proficiency with HCHB (Homecare Homebase) software
• Preferred:
  • Bachelor’s Degree in Nursing (BSN)
  • Case management certification
  • Experience in care management
