Description
Job Summary:
The Travel RN Case Manager is responsible for coordinating and managing patient care across the continuum to ensure optimal clinical outcomes, efficient resource utilization, and safe transitions of care. The RN Case Manager serves as a liaison between patients, healthcare providers, and community resources to facilitate high-quality, patient-centered care in an acute care setting.
Key Responsibilities:
- Perform comprehensive assessments and develop individualized care plans for assigned patients.
- Coordinate care and discharge planning in collaboration with physicians, nurses, and interdisciplinary teams.
- Monitor patient progress and ensure continuity of care through appropriate referrals and follow-ups.
- Evaluate medical necessity, length of stay, and utilization of healthcare resources.
- Serve as a patient advocate to promote access to services and quality outcomes.
- Provide education and support to patients and families regarding post-discharge needs and available community resources.
- Maintain accurate documentation in accordance with facility standards and regulatory requirements.
- Facilitate transitions of care between acute, post-acute, and community settings.
- Collaborate with insurance providers to ensure proper authorization and reimbursement.
Requirements:
- Experience: Minimum 2 years of recent acute medical Case Management experience performing Care Coordination (Required).
- Licensure: Active Registered Nurse (RN) license in the state of assignment.
- Certifications:
- BLS – Basic Life Support (Required).
- ACM or CCM (Accredited Case Manager / Certified Case Manager) certification preferred but not required.
- Floating Expectation: Must be willing to float to Laguna Facility as needed.
- Ratios: 1:25 (Patients per RN Case Manager).
Preferred Qualifications:
- Experience with acute hospital discharge planning and utilization review.
- Strong knowledge of Medicare, Medicaid, and insurance authorization processes.
- Excellent communication, critical thinking, and organizational skills.
- Experience with EMR systems and case management software.
