💙 RN Case Manager – Hospice | Full-Time | Atlanta, GA (In Patient - Virtual)

Your Day-to-Day

• Provide individualized care using the nursing process: assess, plan, implement, evaluate.
• Educate and support families on symptom management, medication administration, and preparing for end-of-life.
• Monitor patient condition and coordinate care across the interdisciplinary team.
• Maintain accurate medical records and ensure compliance with state and federal guidelines.
• Serve as a clinical mentor to fellow RNs, LPNs, and hospice aides.

What You Bring

• Education: Bachelor’s in Nursing or Nursing Diploma (required).
• Licensure/Certifications: RN (Single-State or Multi-State Compact), BLS, Georgia Driver’s License, personal vehicle; CHPN preferred.
• Experience: 2+ years in acute care, hospice, or home care preferred.
• Skills: Emotional intelligence, clinical judgment, excellent communication, and comfort with electronic documentation.
