Overview:
We’re seeking an experienced and compassionate Physical Therapist (PT) to join our home health team in Solano County, California. This role offers an opportunity to provide high-quality, patient-centered care in a flexible home health environment while working closely with an interdisciplinary team.
Key Responsibilities:
- Prioritize and conduct start-of-care visits for new patients.
- Supervise Physical Therapy Assistants (PTAs) and ensure adherence to treatment plans.
- Submit clinical notes and documentation within 48 hours of visits.
- Maintain timely and clear communication with supervisors, intake, and office staff.
- Provide next-day reporting for start-of-care visits.
- Remain responsive during normal business hours.
Schedule & Workload:
- Full-time, 40 hours per week (Saturday–Wednesday)
- Productivity Requirement: 30 points per week (equivalent to ~8 hours per day, 40 hours per week)
- Training: 3 days (1 in-office, 2 in-field)
Requirements:
- Active California PT License
- Minimum of 1 year of Home Health experience
- Familiarity with Kinnser (EMR system) preferred
- Reliable transportation (company car option available)
Compensation & Benefits:
- Mileage reimbursement: $0.655 per mile (from first to last patient)
- Parking and tolls: Reimbursed
- Equipment provided: iPad and Verizon One Talk
