Nurse Case Manager (2 years)

Description

💼 Job Title: Medical Social Worker (MSW/LCSW) – Case Management / Discharge Planning
📍 Location: Laguna, California (within 50-mile radius – locals accepted at lower rate)
🕒 Shift: 5x8-Hour Days (08:00 AM – 4:30 PM)
📅 Schedule: Sunday–Thursday (subject to change based on staffing)
📆 Duration: 13 Weeks
🧭 Orientation: 40 hours

Facility Type: Acute Care Hospital

Requirements

License: Active California LCSW or MSW (pending accepted, must be active by start date).
Experience:
- Minimum 1 full year of acute care hospital experience (required).
- Travel experience required (no first-time travelers).
- Experience in discharge planning and care coordination.
Certifications:
- BLS (required)
- LCSW or MSW (required)
Charting System: Epic (required)
