Position Summary:

The RN Case Manager is responsible for coordinating and managing patient care across the continuum to ensure quality, cost-effective outcomes. The role involves assessing patient needs, developing individualized care plans, facilitating communication among interdisciplinary team members, and ensuring appropriate utilization of resources and services.

Key Responsibilities:

  • Assess patients’ clinical and psychosocial needs to develop, implement, and monitor individualized care plans.
  • Coordinate and oversee all aspects of patient care from admission through discharge and post-discharge follow-up.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care.
  • Educate patients and families about treatment options, resources, and self-care strategies.
  • Monitor and evaluate patient progress and outcomes; modify care plans as needed.
  • Ensure compliance with organizational policies, accreditation standards, and federal/state regulations.
  • Advocate for patients’ needs while balancing clinical and financial outcomes.
  • Document all case management activities accurately and timely in the electronic medical record (EMR).
  • Participate in multidisciplinary team meetings and quality improvement initiatives.
  • Identify barriers to care and coordinate services to overcome them (e.g., transportation, insurance authorization, referrals).

Licensure/Certification:

  • Current Registered Nurse (RN).
  • Case Management certification (CCM, ACM, or similar) preferred.

Experience:

  • Minimum [2] years of clinical nursing experience (medical-surgical, critical care, or home health preferred).
  • Prior experience in case management, utilization review, or discharge planning strongly preferred.
