Description

Care Coordinator RN (Case Management / Discharge Planning)

Location: Bakersfield, CA
Start Date: 12/08/2025
Duration: 13 Weeks
Shift: Days, 8-hour shifts
Guaranteed Hours: 40 hrs/week
Care Setting: Acute – Inpatient
Unit: Care Coordinator (Case Management – Utilization Review)
Charting System: Cerner

Requirements

License: Active RN license (OK to submit)
Experience:
- 3 years RN experience in an acute hospital setting
- Must have prior Care Coordination experience in a CommonSpirit / Dignity / CHI hospital (non-negotiable)
BLS required
BSN not required
Scrubs: Business casual with lab coat
Call: None

Role Overview

Focus: Discharge Planning (not UR)
Setting: Inpatient
Typical patient populations & duties:

Work with all payer types, including uninsured + homeless populations
Perform initial assessments, develop discharge plans, coordinate:
- Nursing homes/SNFs
- Home health
- DME
- Prior authorizations
Daily caseload: 15–28 patients on average
Requires high attention to detail, strong communication, and excellent customer service

Soft Skills the Facility Requires

Excellent customer service
Strong interpersonal & written communication
Ability to multi-task and work independently
Comfortable in a fast-paced environment with shifting priorities
