Setting: Acute Care Hospital
- Division: Case Management (Discharge Planning only)
- Shift: 8-hour Days
- Guaranteed Hours: 40/week
- Contract Length: 13 weeks
Experience Required:
- 3+ years in acute hospital setting
- Prior care coordination experience in a CommonSpirit, Dignity, or CHI facility (must-have)
- License: Active RN license; submission allowed
