Nurse Case Manager (2 years)

Description

Requirements:

2 years acute medical Care Manager experience (care coordination, discharge planning)
Travel experience required – First-timers not accepted
CA RN license (pending accepted, must be active by start)
Experience with HMOs/IPAs, DME coordination, SNF placement, ambulance transport
Strong knowledge of adoption/foster care resources, mental health, homelessness, crisis intervention, Advance Directives, POLST, protective services
