Description
Job Summary – Case Manager (Day Shift, 5x8)
Start: ASAP
The Case Manager is responsible for coordinating high-quality, patient-centered care through utilization review, resource management, treatment planning, financial oversight, and comprehensive discharge planning. This role ensures that patients receive appropriate, efficient, and safe care while meeting all regulatory, documentation, and payor requirements.
Key Responsibilities:
- Conduct utilization reviews and ensure medical necessity for admissions and continued stays.
- Lead and coordinate individualized treatment and discharge plans with the interdisciplinary team.
- Identify patient and family needs, barriers to care, and connect them with hospital and community resources.
- Ensure accurate and timely documentation in the medical record and MedHost UR system.
- Support patient and employee safety in alignment with Select Medical’s Core Values and the 10-Foot Circle of Safety.
- Maintain strong communication with patients, families, physicians, payors, and community agencies.
- Promote payor satisfaction by ensuring timely insurance certifications, reimbursement documentation, and contract compliance.
- Assist in minimizing facility financial risk through accurate verification, billing awareness, and proactive payor management.
- Participate in team conferences, meetings, and facility improvement initiatives.
- Provide coverage for Director of Case Management duties when assigned.
Required Skills & Experience:
- Active licensure in a clinical discipline (RN, RT, or SW).
- Strong working knowledge of insurance, government reimbursement, and payor guidelines.
- Minimum 2 years of Case Management and Discharge Planning experience in a rehabilitation care setting.
- RN background highly preferred.
