Description

Position: Hospice RN Case Manager
Shift: Days | 5x8s | 08:00–16:00
Schedule: Monday–Friday + every 3rd weekend rotation
Contract: 13 Weeks

Requirements

1+ year of Hospice RN Case Management experience (Required)
- Must include Hope Assessment, pain & symptom management
Valid driver’s license & clean driving record
BLS certification
Epic charting experience (preferred/required depending on site
Travel to patient homes & facilities required
