Description
Position Summary
The RN Case Manager is responsible for providing comprehensive care coordination, discharge planning, and utilization management within an acute care setting. The role focuses on ensuring effective patient flow, safe transitions of care, and optimal resource utilization. Candidates must have strong clinical knowledge, communication skills, and the ability to collaborate with interdisciplinary teams in a fast-paced hospital environment. Hospital experience is mandatory.
Key Responsibilities
Care Coordination & Discharge Planning
- Conduct assessments to determine patients’ discharge needs and coordinate appropriate services.
- Collaborate with physicians, nursing, therapy, and ancillary departments to facilitate timely and safe discharge plans.
- Arrange post-acute placement including SNFs, home health, rehab, DME, transportation, hospice, and community services.
- Communicate with patients and families to educate on discharge processes, care needs, and follow-up requirements.
Utilization Review & Resource Management
- Review clinical documentation to ensure appropriate patient status (inpatient, observation, etc.).
- Partner with utilization management to ensure payor criteria are met and avoid denials.
- Monitor patient progress and adjust plans of care to support safe length of stay.
Interdisciplinary Collaboration
- Participate in daily rounds, huddles, and care coordination meetings.
- Serve as a resource for clinical staff regarding discharge barriers, community resources, and regulatory requirements.
- Maintain strong relationships with post-acute providers and internal hospital departments.
Patient Advocacy & Compliance
- Advocate for patient needs while ensuring compliance with federal, state, and hospital regulations.
- Document all assessments, communication, and care plans accurately in the EMR.
- Maintain knowledge of Medicare, Medi-Cal, managed care policies, and regulatory guidelines related to discharge planning and utilization management.
Qualifications
Required:
- Active California RN license.
- Minimum 2 years of acute care RN Case Manager experience performing both care coordination and discharge planning.
- Strong knowledge of hospital workflows, post-acute care levels, and community resources.
- Ability to handle a 1:20 caseload ratio effectively.
- Experience communicating with physicians, interdisciplinary teams, and external agencies.
Preferred:
- EMR experience (EPIC or similar).
- Prior traveler experience (first-time travelers accepted; clinical team will screen).
- Strong utilization management knowledge.
