Job Title: RN Case Manager – Acute Care Hospital

Location: Southwest Houston, TX

Contract Length: 13 Weeks 

Schedule: 8:00 AM – 4:30 PM | Start Date: ASAP

Overview:
Protouch Staffing is seeking a Registered Nurse Case Manager to join the Case Management team at a reputable acute care hospital in Southwest Houston. This local contract role is ideal for a professional passionate about care coordination, utilization review, and discharge planning. You will play a vital role in improving patient outcomes and optimizing healthcare resources.

Responsibilities:

• Support the Director of Case Management in the oversight and execution of case management services.
• Oversee Care Facilitation, Utilization Management, and Discharge Planning across inpatient units.
• Assist in day-to-day departmental operations, manage resource utilization, and support quality improvement initiatives.
• Collaborate with interdisciplinary teams to ensure effective transitions of care and compliance with regulations.
• Analyze and report on utilization and quality metrics, acting on findings to improve outcomes.

Qualifications:

• Valid RN License (Texas or Compact State)
• AHA BLS/CPR Certification
• Prior acute care experience in case management
• Strong communication, coordination, and documentation skills
• TB skin test or chest X-ray
• Titers for Hepatitis B, MMR, and Varicella
• Tdap within the last 10 years
