Job Description: RN Case Manager

Contract Length: 13 weeks
Schedule: 5×8-hour days per week; includes 2–3 weekends during the contract
Licensure: VA RN license accepted; candidates may be submitted with VA license pending

Position Summary

We are seeking an experienced RN Case Manager to support inpatient case management within an acute care hospital setting. The ideal candidate will have strong pediatric case management experience and a solid background in coordinating care and discharge planning for hospitalized patients.

Key Responsibilities

• Manage patient caseload with emphasis on pediatric populations
• Perform assessments, develop care plans, and coordinate services across the continuum of care
• Collaborate with physicians, nursing staff, therapists, social workers, and other interdisciplinary team members
• Facilitate safe and timely discharge planning, including referrals and resource coordination
• Ensure compliance with hospital standards, documentation requirements, and regulatory guidelines
• Maintain accurate and timely EMR documentation
• Participate in required weekend rotations (2–3 weekends over the 13-week assignment)

Required Experience & Qualifications

• Active RN license (VA license or ability to obtain; submissions allowed with VA license pending)
• Pediatric case management experience (required)
• Acute care hospital inpatient case management experience (required)
• Strong clinical assessment, critical-thinking, and communication skills
• Experience with hospital EMR systems

Preferred Qualifications

• Prior travel/contract RN CM experience
• Knowledge of discharge planning standards, utilization management, and regulatory requirements
