Location- 7500 Hospital Dr, Sacramento, CA 95823
Shift- Days (8-hour shifts) + every 3rd weekend; first to float
Contract: 13 weeks
Pay Term- Weekly (By Every Friday)
Benefits: Paid orientation & sick leave (13)

Job Description:

Unit & Responsibilities

Unit: Care Coordination (Case Management)
Unit Type: Case Management (Utilization Review)
Division: Nursing
Care Setting: Acute Care
Patient Ratio: 1:20
Qualifications
Experience: Minimum 3 years hospital experience with acute care coordination; must have prior CA Dignity/CommonSpirit acute care experience
Education: BSN not required
Licenses / Certifications: RN license required; BLS required
Orientation: 12 hours required, plus floor-specific training (4-day webinar + 2-day preceptor)
References: 2 supervisor references required (1 within 12 months, 1 within 3 years)
Responsibilities:
Perform acute care coordination, discharge planning, and utilization review
Float to cover all adult areas of the hospital (women’s health experience required to float there)
Complete charting in Cerner, Curaspan, Midas, InterCall, and Microsoft Office
Collaborate with interdisciplinary teams to ensure high-quality patient care
Maintain excellent attendance, adaptability, and professional demeanor
