Location- 7500 Hospital Dr, Sacramento, CA 95823
Shift- Days (8-hour shifts) + every 3rd weekend; first to float
Contract: 13 weeks
Pay Term- Weekly (By Every Friday)
Benefits: Paid orientation & sick leave (13)

Job Description:
Unit & Responsibilities
- Unit: Care Coordination (Case Management)
- Unit Type: Case Management (Utilization Review)
- Division: Nursing
- Care Setting: Acute Care
Patient Ratio: 1:20
Qualifications
- Experience: Minimum 3 years hospital experience with acute care coordination; must have prior CA Dignity/CommonSpirit acute care experience
- Education: BSN not required
- Licenses / Certifications: RN license required; BLS required
- Orientation: 12 hours required, plus floor-specific training (4-day webinar + 2-day preceptor)
References: 2 supervisor references required (1 within 12 months, 1 within 3 years)
- Responsibilities:
- Perform acute care coordination, discharge planning, and utilization review
- Float to cover all adult areas of the hospital (women’s health experience required to float there)
- Complete charting in Cerner, Curaspan, Midas, InterCall, and Microsoft Office
- Collaborate with interdisciplinary teams to ensure high-quality patient care
- Maintain excellent attendance, adaptability, and professional demeanor
