About the Role

We are seeking an experienced Case Manager RN to join our team for a travel assignment in California. This role requires strong inpatient case management expertise, knowledge of HMO and Medicare guidelines, and proficiency in Epic charting. The Case Manager RN will play a critical role in coordinating patient care across inpatient, home health, and skilled nursing facility (SNF) settings.

Key Responsibilities

• Provide comprehensive inpatient case management services.
• Coordinate patient care transitions across hospital, home health, and SNF settings.
• Ensure compliance with HMO and Medicare guidelines.
• Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomes.
• Document patient care and case management activities using Epic EMR.
• Float within scope to meet facility needs as required.

Requirements

• Experience: Minimum of 3 years inpatient case management experience.
• Licensure: Active California RN license (pending licenses not accepted).
• Knowledge: HMO and Medicare for inpatient, home health, and SNF settings.
• Charting: Epic experience required.
• Certifications: BLS required.
• Travelers: First-time travelers accepted.
• Locals: Accepted.
• RTO Restrictions: No RTO permitted during the first 2 weeks of assignment.
