Description
Job Overview
A skilled Physical Therapist Assistant is needed to provide rehabilitative support in a long-term care/post-acute environment. The PTA will work under the supervision of a Physical Therapist to deliver resident-centered care focused on improving mobility, functional ability, and overall quality of life. This role requires strong communication, consistent documentation, and the ability to collaborate effectively with the interdisciplinary team.
Responsibilities
- Carry out treatment plans established by the supervising Physical Therapist.
- Provide therapeutic exercises, transfer and gait training, balance activities, and functional mobility interventions.
- Track and document patient progress, response to therapy, and notable changes in condition.
- Educate residents and caregivers on safe mobility techniques and rehabilitation goals.
- Partner with nursing, therapy, and facility teams to ensure coordinated, high-quality care delivery.
- Maintain accurate, timely documentation in accordance with facility and regulatory guidelines.
Requirements
- Active Oregon PTA license (or eligibility prior to start).
- Graduate of an accredited Physical Therapist Assistant program.
- Previous experience in LTC/SNF strongly preferred.
- Strong clinical judgment, communication skills, and ability to work both independently and collaboratively.
Setting: Long-Term Care / Post-Acute
Shift: Variable Days | 8-hour shifts
Hours: 36 hours/week
