Job Overview
A Physical Therapist is needed to provide evaluation and treatment services within a skilled nursing and long-term care setting. The PT will assess resident needs, craft individualized plans of care, and deliver evidence-based therapeutic interventions to support mobility, safety, and functional independence. This role requires strong clinical reasoning, timely documentation, and effective collaboration with the interdisciplinary team.

Responsibilities

• Complete thorough PT evaluations and functional assessments.
• Develop and implement individualized treatment plans aligned with resident goals and medical presentation.
• Provide therapeutic exercises, gait and transfer training, balance interventions, strengthening, and functional mobility work.
• Document evaluations, daily notes, progress updates, and reassessments in accordance with facility and regulatory standards.
• Collaborate with nursing, OT, SLP, and interdisciplinary partners to promote coordinated, high-quality care.
• Educate residents and caregivers on safe mobility, home exercise programs, and functional strategies.
• Adjust care plans based on progress and evolving clinical needs.

Requirements

• Active South Carolina PT license (or ability to obtain prior to start).
• Degree in Physical Therapy from an accredited program.
• SNF/LTC or geriatric rehabilitation experience preferred; strong clinicians will also be considered.
• Strong documentation, communication, and clinical decision-making skills.
• Ability to work independently and manage a full caseload effectively.
