ICU / Critical Care RN – CVICU / MSICU / PCU (Traveler)

Pre-Hire Requirements

  • Clinician must take and pass a dysrhythmia exam (β‰₯80%) within two attempts prior to signing
  • If clinician fails twice, they will be DNU for CS AZ and Sacramento for 6 months
  • Radius Rule: 50 miles
  • Candidates must not have been FT/PT/PRN with any CommonSpirit facility (including CHI or Dignity) within the past 12 months

Unit Overview

Critical Care Service Line (All Under One Service):

  • CVICU – 12 beds
  • COVID ICU – 8 beds
  • MS ICU – 8 beds
  • PCU – 16 beds (up to 20 with double occupancy)

Staffing & Ratios

  • ICU: 1:1–1:2 (based on acuity/staffing)
  • PCU: 1:3–1:4
  • Charge Nurses:
    • Typically 2 charge nurses covering ICUs and PCU
    • Do Rapid Response & Code Blue
    • Take assignments only as last resort
  • House Supervisor: 24/7 (with rare exceptions)
  • Nurse Aides: None

Provider & Support

  • Intensivists: In-house 24/7 (West campus)
  • NP support
  • Respiratory Therapy: Covers all areas (not dedicated ICU RT 24/7)

Required Certifications

  • BLS
  • ACLS

Required Experience & Skills

Travelers will work all ICU/PCU areas, so strong critical care experience is required.

Must Have:

  • Strong EKG interpretation
  • ABG interpretation
  • Vent weaning and troubleshooting
  • Ability to titrate critical care drips
  • Experience floating between ICU/PCU

Strongly Preferred:

  • IABP
  • Prismaflex CRRT

Skills Checklist (Travelers should check 3–4):

  • EVD
  • Chest tubes
  • Arterial lines
  • Swan-Ganz

Patient Population / Diagnoses

  • COVID (manual proning)
  • Post-surgical (high volume)
  • Cardiac surgical patients
  • STEMI, PCI, TAVR, CEA
  • Impella, IABP, VADs (some)
  • DKA, sepsis, ARF, rhabdo
  • CRRT
  • Detox
  • Low-level neuro
  • Fresh open hearts (if experienced)

Drips & Medications

  • All critical care drips including but not limited to:
    • Pressors (Levophed, Dopamine)
    • Sedation (Propofol)
    • Paralytics
    • Antiarrhythmics
    • Anticoagulants (Heparin, Argatroban)

Monitoring & Equipment

  • Tele: Nihon Kohden
  • RNs read their own strips
  • Equipment:
    • Vents (nurses expected to troubleshoot & assist with weaning)
    • BiPAP / CPAP
    • HFNC / Vapotherm
    • Glidescopes (familiarity required)

Scheduling

  • Shifts: 7a–7p / 7p–7a
  • Schedules built 4 weeks in advance
  • Limited self-scheduling (manager finalized)
  • Block scheduling typically not allowed
  • First schedule fills open needs

Weekends & Holidays

  • Weekends: 4 weekend shifts per pay period
    • Nights: Fri/Sat or Sat/Sun/Mon
    • Days: Sat/Sun or Sun/Mon/Tues
  • Holidays:
    • Winter holidays required: 1 holiday + eve of another
    • Not required to work all holidays

Call & Floating

  • On-call: Low census only (approx. once every 6 weeks)
  • Floating:
    • PCU & MST
    • EC ICU/PCU as needed per staffing rotation

Orientation

  • 8 hours general hospital orientation
  • 1 Γ— 12-hour shift with preceptor
  • 1 Γ— 12-hour shift with buddy
  • Additional orientation for specialty skills (CRRT, Open Heart, etc.) as needed

Scrubs

  • Any professional scrub color
  • No camo
  • Transitioning to Navy in October
