Description

Job Overview
A Skilled Nursing Facility in Paonia, Colorado is seeking an experienced Speech-Language Pathologist for a 13-week travel assignment. This role is ideal for an SLP who is comfortable working with a geriatric and post-acute population, with a strong focus on communication, cognition, and swallowing disorders.

Responsibilities

Evaluate and treat patients with speech, language, cognitive, and swallowing disorders
Develop and implement individualized treatment plans
Perform dysphagia assessments and recommend appropriate diet modifications
Complete evaluations, progress notes, and discharge summaries accurately and timely
Educate patients, families, and caregivers on therapy goals and strategies
Collaborate with nursing staff and interdisciplinary care teams

Requirements

Active Colorado Speech-Language Pathologist license
Skilled Nursing Facility or post-acute experience preferred
Dysphagia management experience strongly preferred
Ability to manage an independent caseload
Strong documentation, communication, and clinical judgment skills
