Description

Job Overview
A Skilled Nursing Facility in Barrett, Minnesota is seeking an experienced Occupational Therapist for a 13-week travel assignment. This role is ideal for an OT who is flexible, comfortable with census-driven scheduling, and experienced in post-acute and geriatric rehabilitation.

Responsibilities

Evaluate and treat residents in a skilled nursing facility setting
Develop and implement individualized treatment plans
Address ADLs, IADLs, upper extremity function, and functional mobility
Complete evaluations, daily treatment notes, and discharge summaries accurately and timely
Educate residents and caregivers on exercises, safety, and adaptive strategies
Collaborate with nursing staff and interdisciplinary care teams

Requirements

Active Minnesota Occupational Therapist license
Skilled nursing or post-acute experience preferred
Strong geriatric and functional rehabilitation skills
Ability to adapt to fluctuating census and variable scheduling
Strong communication, organization, and documentation skills
