Job Title: Registered Nurse - Case Manager
Location: Mission Viejo, CA 92691
Duration: 13-week contract with possibility of extension
Tentative Start Date: 01/05/2026
Schedule: 5x8-Hour (8:00 AM - 4:30 PM), Every other weekend required
Travel Pay Rate: $3139.00 per week ($1200.00 taxable + $1939.00 Non-Taxable/Stipend)

What You'll Do:

• Manage a patient caseload of up to 1:25 in an acute care setting
• Perform comprehensive care coordination and discharge planning
• Assess patients and develop individualized treatment plans in collaboration with the interdisciplinary team
• Independently arrange DME, SNF placements, and ambulance transport
• Organize and participate in family meetings regarding patient care
• Manage appeals and denials and navigate HMO, IPA, and managed care plans
• Support patients with complex social needs including homelessness, mental health crises, drug/alcohol abuse, and adoption/foster care
• Facilitate Advance Directives, POLST, and protective services processes
• Note: This position does NOT require Utilization Review (UR) responsibilities

Requirements:

• Active California RN License (Pending license accepted, but must be in hand at start)
• 2+ years of Acute Care Case Management experience required
• Previous Travel Nursing experience is REQUIRED (Hardstop: No first-time travelers)
• Epic Charting experience is REQUIRED
• Must be willing to float to the Laguna Beach facility as needed
• Proficiency in independently arranging DME, SNF, and transport
