Description

Job Overview
A long-term care facility in Belle Fourche, South Dakota is seeking an experienced Physical Therapist for a 13-week travel assignment. This role is ideal for a PT who is comfortable working with a geriatric and post-acute population and values a flexible day-shift schedule.

Responsibilities

Evaluate and treat residents in a long-term care setting
Develop and implement individualized treatment plans
Focus on improving mobility, strength, balance, and functional independence
Complete evaluations, daily treatment notes, and discharge summaries accurately and timely
Educate residents and caregivers on exercises, mobility strategies, and safety
Collaborate with nursing staff and interdisciplinary care teams

Requirements

Active South Dakota Physical Therapist license
Long-term care or post-acute experience preferred
Strong geriatric rehabilitation skills
Ability to manage a caseload independently
Strong communication, organization, and documentation skills

Additional Details

Contract Length: 13 weeks
Schedule: Day shifts
Shift Length: Combination of 8-hour and 10-hour shifts
Guaranteed Hours: 36 hours per week
Setting: Long-Term Care / Nursing Home
Location: Belle Fourche, South Dakota

13-week Travel PT assignment in a long-term care setting in Belle Fourche, SD. Day shifts with a mix of 8- and 10-hour shifts and 36 guaranteed hours per week. Active SD PT license required. LTC/post-acute experience preferred.
