Hospice Case Manager
Clincial Interviews
Certs Required: BLS
Experience Required: 1 year. Will accept first time traveler as long as they ahve 1 year of Home Health exp. Must have patient/family education, provider communication, disease management, Troubleshoot/knowledge of pleurx drains, nephrostomy tubes, trachs, PCA’s, gastric suctioning; will not perform these tasks, just knowledge on troubleshooting – will be trained on site if needed.
Patient Ratios - RN Case Manager: Caseload is 22-25 a week; 6 visits a day – if the traveler is unable to keep up with the caseload, please let leadership know
Patient Types: High acuity patients: pleurx drains, PCA pumps, IV ABX, TPN, Foleys, wounds, wound VAC, chest tubes, various drains. Staff will primarily be seeing patients in the community setting, i.e. their home, the home of a family member, Assisted Living Facilities, Group Homes, Nursing Homes.
