Description
Job Summary (List Format):
- Candidates cannot have an AZ home address on their driver’s license (must provide copy with submission).
- Banner-specific cover page must be completed and accurate, including all experienced departments and locations the traveler is willing to work in.
- Indicate all locations, departments, and shifts the candidate is open to on submission/cover page.
- Start date should be on or shortly after the listed order start date.
- Requested Time Off (RTO):
- Must be less than 7 days.
- Not approved in the first week or during holidays.
- All RTO must be listed on the cover page.
- Candidates must be pre-closed and ready to accept the offer.
- Notify if candidate withdraws or becomes unavailable.
- Minimum requirements:
- 24 months of recent Level I Trauma Med Surg experience (REQUIRED).
- No license hits (REQUIRED).
- Active Arizona RN license (verification <30 days old).
- Active ACLS & BLS certifications (AHA or Red Cross), included with submission.
- Flu vaccination status documented; declinations possible with proper forms.
- Preferred: Previous travel experience.
- Required with submission:
- Completed Banner cover page.
- Resume including all facilities, facility size, beds, trauma level, and units.
- Skills checklist (less than 1 year old).
- Copies of certifications and active license.
- Driver’s license copy.
- Compliance: Must meet Banner's strict background requirements.
- Weekend/on-call: Schedule determined by facility.
- State License: Must hold/maintain an active Arizona RN license at submission.
- Certification requirements: ACLS (AHA), BLS (AHA).
- Flu vaccination: Required or declination form submitted.
