MUST HAVE EXPERIENCE: Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience. Experience must include at least 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge EPIC CHARTING experience is REQUIRED 5x8hr shifts non-negotiable General Certification Requirements (BLS, etc.) BLS, RN Case Manager Other Certifications/Specialty certifications requirements? CCM preferred but not required Prefer RN, BSN, *Would consider ADN . Department: CM - General Case Manager/Discharge planning
